The Vistas at Bettendorf is currently seeking a Full-Time MDS Coordinator/Registered Nurse (RN) or Licensed Practical Nurse (LPN) who possesses excellent organizational, leadership, assessment and documentation skills. MDS experience in a long-term care setting Required, and a Nursing license in the state of Iowa.

Responsibilities:

• Ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for our patients
• Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs

Requirements/Qualifications:

• Must possess a current, unencumbered, active state license to practice as an RN/LPN
• 1yr minimum MDS experience required
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• You must be qualified, compassionate, and dedicated to a job well done
• Utilize clinical documentation improvement strategies to ensure accurate coding and billing
• Collaborate with the interdisciplinary team to develop individualized care plans
• Monitor and update patient records in compliance with regulatory requirements
• Provide education and training to staff on MDS completion and documentation best practices
• Prior experience in HEDIS reporting and medical documentation
• Knowledge of hospice care, case management, and utilization management practices
• Understanding of Diagnosis Related Groups (DRG) and reimbursement methodologies
