Job Title: Nurse - RN or LPN (Night Shift)

Location: Elmore Assisted Living - Elmore, MN and Nagel Assisted Living – Waconia, MN

Schedule:  Full-Time Nights OR Part-Time Weeknights and Weekends

Pay: $25-$30/hour (LPN) | $35-$45/hour (RN)

Position Overview: Sunflower Communities is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) with experience in geriatric care to join our team. Successful applicants will have prior experience in caring for the older adult population.

Nurse - RN / LPN Overview:

• Oversee the coordination and plan of residents’ care, ensuring care practices have each resident’s best interests in mind
  
• Complete clinical assessments of residents’ health status and document appropriately
  
• Monitor the status of chronic illnesses and manage the administration of medications
  
• Coordinate with external health care providers and internal team members
  
• Interact with residents’ families and loved ones, reporting changes in their health or care plan
  

Nurse – RN / LPN Qualifications:

• Licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota
  
• Experience with Electronic Medical Records (EMR) software
  
• Prior nursing experience in a SNF, AL, TCU, or Home Health setting is ideal
  
• Demonstrated ability to work autonomously
  
• Excellent at planning, organizing, and communicating
